36. Thermometers provided & accurate
- comments: observed no probe stem thermometer for taking food temperatures (milk) upon receiving the products. Must provide. Priority foundation 7-38-005, citation issued

38. Insects, rodents, & animals not present
- comments: observed an opening at the door located behind the service counter. Instructed to seal said opening to prevent potential entry points for pest and maintain.

- comments: observed evidence of pest activity. Observed about 80-100 mice droppings on site. Observed mice droppings on the windowsill and floor near the rear area where the mens undergardments are stored, along the hallway walls where the black bins and employee toilet rooms are located, on top of and under the shelving units where prepackged foods such as: cookies, peanuts, cereal, boxes of chips are stored, in the lower compartments behind the service desk. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation 7-38-020(a), citation issued

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed both of the all gender toilet rooms on site with no lided receptacle. Instructed to provide and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed the floors in employee toilet room closest to the end of the hallway with paper towel and not maintained. Instructed to clean and maintain said floor.
