Report date: Sep 20, 202310. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hot running water at the handwashing sink in all gender toilet room near the office. Temperature of water was 74f at the time of inspection. Instructed to have hot (100-120f) and cold running at all hand wash sinks. Priority violation 7-38-030(c), citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed pink and black mold like substance throughout interior of the ice machine. Instructed to clean, sanitize and maintain. Ice is used for human consumption. Priority foundation violation 7-38-005, citation issued.
38. Insects, rodents, & animals not present
- comments: observed the rear exit door near the ice machine not rodent proofed at the bottom. Instructed to rodent proof said door and maintain.

- comments: observed pest infestation. Observed approximately 200-300+ mice droppings throughout the facility in the following areas: on the floor where the ice machine is stored, on the floor and shelving units in the storage area near the office and the food prep area. On the floors in the food prep area under the three compartment sink, on the sides of the tables, inside a damaged wall base, and inside the lower compartment of the deep-frying units. On the floors, shelving units, and the items stored in the hot water tank closet and in the 'employees only' closet. In the women toilet room in the shelving unit under the handwashing sink and men toilet room on the floor next to the garbage, both said toilet rooms were observed near the 'employee only' storage closet. Observed in all three bars on the floors, inside of the cabinets, on the tabletops where alcohol and registers were stored, on the shelving units where cups, and shot glasses were stored, and on the table with the 'reserved for' sign across form the bar near the office. Also, observed 30 small flies behind the two bars across from the dance floor. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority violation 7-38-020(a), citation issued

- comments: observed that pest control logbook is missing required information. Missing record of what service was performed and findings during the pest control service. Also missing copy of service ticket that sets forth pest control idph license number. Must provide and maintain all required pest control records and documents. Priority foundation violation 7-38-020 (c) citation issued
39. Contamination prevented during food preparation, storage & display
- comments: observed all the ice bins behind the bars not covered. Instructed to provide lids to cover the said ice bins to prevent contamination.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no sanitizer test strip kit on-site at the time of inspection. Instructed to have a test strip kit on site at all times to monitor the strength of the sanitizer properly. Priority foundation 7-38-005, citation issued

- comments: observed two missing sink stoppers for the three comaprtment sink. Instructed to provide and maintain

- comments: observed grease and food debris on top of the stove burners in the food prep area. Instructed to clean and maintain equipment.
51. Plumbing installed; proper backflow devices
- comments: observed no mops sink on site. Instructed to install atleast one service sink or curbed cleaning facility equipped with floor drain a floor drain shall be provided and conveniently located for cleaning of mops or similar wet floor cleaning tools and for disposal of mop water or similar liquid waste.

- comments: observed a hole in the wall under the three compartment sink and a hole in the floor at the sky box bar. Also observed a damaged wall base at the bottom of the floor near the food prep table in the food prep area. Instructed to seal said holes and repair the damaged wall base so that all said surfaces are smooth and easily cleanable and maintain.

- comments: observed the floors under the three compartment sink and cooking equipment under the ventilation hood food in the food prep area with severe grease up and not maintained. Also observed the floors in the sky box bar with debris on the sides and underneath the equipment. Instructed to clean said floors and maintain.

- comments: observed unnecessary items stored in the hot water tank closet and the 'employee only' closet. Also, observed the cabinets located at the bar near the 'employee only' closet with clutter and also clutter on the floor and corner at the rear of the same said bar. The storage closet near the office was also had clutter. Instructed to remove all unecessary items and declutter all said areas and maintain.

- comments: observed a blown out light in the food prep area. Instructed replace said light and maintain.
