- comments: observed facility with womiting and diarrhea clean up kit and no corresponding clean up procedure for responding to vomiting and diarrheal events. Instructed to provide and maintain appropriate supplies and the written procedure that corresponds. Must keep both the policy and supplies on site at all times. Priority foundation violation #7-38-005. Citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: observed no disclosure and reminder on the printed menu for all animal food products that can be ordered raw or undercooked. Instructed to provide a disclosure on the menu for all items (eggs any style, steak, fish) that can be ordered undercooked or raw and add a reminder on the menu that reads: *these items can be ordered undercooked or raw. Consuming undercooked or raw foods can increase your risk of foodborne illness. (refer to cdph consumer advisory handout) provided consumer advisory signage priority foundation violation #7-38-005. Citation issued.

37. Food properly labeled; original container
- comments: noted bulk seasonings in containers on prep table in kitchen. Instructed to provide labels of common name on items removed from its original container.

38. Insects, rodents, & animals not present
- comments: observed pest activity with approx. 10 roached seen during inspection. Roach activity was noted in the hot water closet between the rear dry storage and additional cooling units in the rear. Noted small roaches on the cashier's register in front. Instructed to consult pest control as additional services are needed to minimize or eliminate activity before re-inspection. Priority foundation violation #7-38-020(a). Citation issued.
