- comments: 2-501. 11 found no procedure/plan on-site and the kit not complete, missing eye protection,disposable apron,disposable bags, mop,etc. For responding to vomiting and diarrheal events. Instructed facility to maintain any appropriate supplies on site. Priority foundation violation 7-38-005, no citation issued

8. Hands clean & properly washed
- comments: observed employee handling without gloved hands partially cooked food that he took out from reach-in cooler and transfer products to a serving plater. Information given to manager on handwashing and glove procedure. Manager refers information to employee. Presently procedure is followed priority violation:7-38-010, citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: no soap provided at handwashing sink in prep,dishroom and small bar area. Soap was provided by the manager upon my request. Priority foundation violation:7-38-030,citation issued

- comments: no back flow prevention device provided at water line of ice machine. Must install so back flow prevention device may be located to be serviced and maintain or know the location

- comments: must not use cardboard boxes as matting for floors inside the walk-in cooler. Matting must be washable/cleanable.

- comments: previous core violations not corrected from 11-3-2020,report #2454166 #43)3-304. 12 noted serving plates on shelves at the kitchen prep area and dry storage area not properly stored. Instructed to invert all serving plates to prevent dust accumulation. #47)4-202. 16 noted cardboard boxes used to line shelves at the kitchen prep area and inside refrigeration units. Instructed to clean and remove all cardboard boxes to prevent hiding places for pest. #53)5-501. 17 noted wash rooms on premises with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women wash rooms and unisex wash rooms #55) 6-501. 114 noted heavy clutter throughout rooms used for storage on shelves, floors unused equipment. Instructed to detail clean, organize and remove all unnecessary or unused articles and equipment to prevent hiding places for pest. Priority foundation violations:7-42-090,no citation issued
