- comments: instructed to install hand washing sink at the basement dish machine area with hot and cold running water, soap and paper towels. Priority foundation violation.

- comments: 4-101. 19---must not use milk crates as a means of elevation for various items throughout the facility. Shelving must leave floors accessible for cleaning and monitoring. : 4-204. 12--- cooks line tall reach-in cooler with a missing/broken door handle with exposed insulation. Must replace. ----door has been repaired with duct tape. It must be a smooth, cleanable repair. 4-501. 11--- torn rubber door gaskets on several cooler doors on the cooks line and the box freezer in the basement. Must replace.
