- comments: previous core violations not corrected from august 23, 2021 report #2528381 #16 4-602. 11(b-e) observed all woks and some cooking utensils with accumulated food and grease debris. Instructed to clean and sanitize. #37 3-302. 12 observed spices out of the original packaging and not labeled. Instructed to label all foods that are out of the original packaging. #45 4-502. 13 observed cardboard being reused at a shelf liner in the freezers located in the basement and as a floor liner in the food prep area. Instructed not to re-use cardboard and make all surfaces smooth and easily cleanable. #49 4-601. 11(c) observed the following equipment in need of cleaning: clean exterior surface of cooking burner, fryer bottom and exterior, stove, prep tables, refrigeration units and vents above the cooking equipment. Observed accumulated ice debris inside the freezers located in the basement. Observed rust on the shelves inside of the walk in cooler. Instructed to remove rust or replace shelves. #53 5-501. 17 observed no covered receptacle inside of the women's toilet room. Instructed to provide. #55 6-501. 12 observed floors with excessive accumulated dirt, food and grease debris in the basement and the food prep area. Observed ceiling with food splatter and observed accumulated grease and food debris inside of the fryers and around the wok station. Instructed to clean and sanitize all and maintain. Instructed to correct and maintain clean at all times. Priority foundation 7-42-090 citation issued
