10. Adequate handwashing sinks properly supplied and accessible
- comments: observed improper maximum water temperature of 137. 0f at handwashing sink in employee all gender washroom. Instructed a handwashing sink shall be equipped to provide water at a maximum temperature of 120f through a mixing valve or combination faucet. Priority foundation violation 7-38-030(c) citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed containers and equipment stored on prep table underneath stained sewer line with damaged wrapping in basement. Instructed to relocate all containers and equipment stored on prep table in basement to prevent contamination from sewer line.

- comments: observed boxes stored and containers stored on floor in basement and observed milk crates used throughout facility for elevation. Instructed to eliminate usage of milk crates for elevation and instructed to provided adequate shelving units to elevate all food and nonfood items at least six inches off of floor for ease of cleanability. Observed rust on shelf next to three compartment sink. Instructed to remove rust on shelf next to three compartment sink, nonfood contact surfaces of equipment shall be constructed of a corrosion resistant, nonabsorbent and smooth material.

- comments: observed damaged gasket on left door of small three door cold holding unit in front prep area. Instructed to repair or replace damaged gasket on left door of small three door cold holding unit in front prep area and maintain.

- comments: observed small upper pipe on east wall near sewer lines leaking onto floor in basement. Instructed to repair said leaking pipe and maintain.

- comments: observed stained ceiling tiles throughout front prep area. Instructed to replace all stained ceiling tiles throughout facility and maintain. Observed upper wall covered in tape above three compartment sink and observed damaged tile above three compartment sink. Instructed to remove tape from wall and smoothly repair damaged tile above three compartment sink for ease of cleanability.

- comments: observe dust on condensor fan covers in walk in cooler. Instructed to clean condensor fan covers in walk in cooler and maintain.
