10. Adequate handwashing sinks properly supplied and accessible
- comments: 6-301. 14 observed no hand washing sign located at the food prep area hand washing sinks and in the men's toilet room. Instructed to provide.

16. Food-contact surfaces: cleaned & sanitized
- comments: 4-602. 11(b-e) observed all food contact surfaces with accumulated debris. Instructed to clean and sanitize.

23. Proper date marking and disposition
- comments: 3-501. 17 observed time/temperature control for safety ready to eat foods (cooked chicken, cooked beef) without labels containing the discard dates. Manager provided labels during the inspection. Instructed to always date mark all tcs rte foods that are held for more than 24 hours in the refrigerators or walk in cooler. Priority foundation. Citation issued. 7-38-005.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of rodent activity. Observed about 50 rodent droppings inside of the dry storage closet in the rear, about 20 rodent droppings located by the dry storage shelves, about 20 rodent droppings under the low temperature dish machine and about 20 rodent droppings on the bottom shelf prep tables located across from the cooking equipment. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued. 7-38-020(a).

45. Single-use/single-service articles: properly stored & used
- comments: 4-502. 13 observed foil being used as a shelf liner at the bottom shelves of all prep tables. Instructed to remove and make all surfaces smooth and easily cleanable.

49. Non-food/food contact surfaces clean
- comments: 4-601. 11(c) observed excessive accumulated food and grease debris in between and inside of all cooking equipments. Instructed to clean all.

51. Plumbing installed; proper backflow devices
- comments: 5-205. 15(a) observed hand washing sink not repaired according to law and without hot and cold running water, also without a drain pipe under the hand washing sink. Instructed to always maintain and repair all hand washing sinks according to law. Plumber arrived during the inspection and by the end of the inspection hot and cold running water was restored and drain pipe replaced. Priority violation. Citation issued. 7-38-030(c).

- comments: 6-201. 11 observed a hole in the wall under the rear hand washing sink. Instructed to make all surfaces smooth and easily cleanable.

- comments: 6-501. 12 observed excessive accumulated food, dirt and grease debris under and behind all refrigeration units, under and behind all shelves and prep tables located in the food prep area. Observed accumulated dust debris on the ceilings in the food prep area. Instructed to clean and sanitize all.

- comments: 6-501. 114 observed excessive clutter in the dry storage closet and in the electrical closet and items stored directly on the floor in the food prep area and in both closets. Instructed to remove unnecessary clutter and elevate all items at least 6 inches from the ground.
