- comments: observed no valid city of chicago sanitation certificate on site at time of inspection. Instructed person in charge to obtain sanitation training and retain certification on site at all times, as well as certified manager physically on site. Priority foundation 7-38-012. Citation issued.
22. Proper cold holding temperatures
- comments: observed tcs foods (chicken pieces) at improper cold holding temperatures ranging from 58. 9f-63. 5f. Instructed person in charge that all cold held food must be at 41f or below. Pic voluntarily denatured and discarded #15 at a cost of $60. Prioriy violation 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed no current pest control log book on the premises. Instructed person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year and the reports must be maintained in the log book. Instructed to provide a current pest control log book and pest inspection reports. Priority foundation 7-38-020(c). Citation issued.

- comments: observed outside waste receptacle over flowing at time of inspection. Instructed person in charge that outside dumpster must be maintained at all times. #7-38-020(b). Citation issued.
