- comments: 4-601. 11(a) observed a black substance inside located by the ice maker and dripping onto ice that is used for human consumption. Instructed to remove and discard contaminated ice and clean and sanitize the ice machine. Priority foundation. Citation issued 7-38-005.

25. Consumer advisory provided for raw/undercooked food
- comments: 3-603. 11 observed various menu items that contain an undercooked egg (sunny side up eggs) and without a disclosure nor a reminder on the menu for these items. Instructed to provide a disclosure and a reminder on the menu for all items that contain undercooked eggs. Priority foundation 7-38-005.

36. Thermometers provided & accurate
- comments: 4-302. 12 observed no metal stem thermometer on site to properly take temperatures of cold and hot tcs foods. Instructed to provide at all times. Priority foundation. Citation issued 7-38-005.

- comments: 3-302. 12 observed dry storage and spice containers not labeled with the product names. Instructed to provide for all.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed no up-to-date pest control log book on site with a most recent service report; at the time of the inspection, the facility presented a pest control service contract that was not signed by the pest control company and a service report from january 2023. Instructed to provide a pest control logbook with service report anytime after 5/15/23. Priority foundation. Citation issued 7-38-020(c).

45. Single-use/single-service articles: properly stored & used
- comments: 4-904. 11 observed utensils (spoons, metal chopsticks) being preset on customer dining tables without protection from possible contamination. Instructed not to preset the tables or set the tables upon customer arrival or provide adequate protection for tableware.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: 4-302. 14 observed no chemical test strips on site properly to test the sanitizing compartment of the 3-compartment sink. Instructed to provide proper test strips for the sanitizer on site which is bleach. Priority foundation. Citation issued 7-38-005.

- comments: 2-102. 13 observed no food handler certificate provided for the female dishwasher. Instructed to meet requirements for all food handlers/dishwashers.
