- comments: pic doesn't have a food managers certificate. Must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty nor a food managers certificate posted to view while tcs foods are being prepared,handled and served such (chicken,fish,cole slaw,etc)must provide and be on site at all times. (priority foundation 7-38-012)

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

36. Thermometers provided & accurate
- comments: must provide a thermometer for dessert cooler.

- comments: must label food storage containers when food is not in original package.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 10 live large flies and appx. 40 or more live small flies flying around premises,on walls and ceilings. Recommended to have a pest control operator to service premises on or after 9/24/20. (priority foundation 7-38-020(a)

- comments: must store belongings properly away from paper goods (on & underneath shelving units)in rear area.

- comments: must store wiping cloths in a sanitizing solution when not in use.

- comments: must invert multi-use utensils on dish storage shelves.

- comments: must clean grease and food debris build up from grill table,fryer cabinets,prep tables,interior & exterior of all coolers,all unused equipment in rear storage area.

- comments: must provide a self closing device on toilet room door that leads into prep & dish washing area.
