- comments: observed no certified manager on duty while tcs items such as curry and chicken were being prepared. Instructed to maintain employee holding city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no verifiable health policy on site at time of inspection. Left template and instructed to maintain health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. No citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no kit and procedure on site for responding to vomit and diarrhea. Left one pager and instructed to maintain procedure and kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. No citation issued.

- comments: observed no paper towels at exposed hand sink located in food prep area. Instructed to provide. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed the following tcs items at the stated improper temperatures: prepared chicken on counter in server area at 67. 8f and curry sauce in prep cooler at 47. 8f. Management voluntarily discarded and denatured approximately 30lbs of product valued at approximately $75. Instructed to maintain all tcs items at or below 41f at all times. Priority violation 7-38-005. Citation issued.

- comments: observed no metal stem thermometer on site at time of inspection for taking internal temperatures of food. Instructed to provide. Priority foundation violation 7-38-005. Citation issued.

- comments: observed no covered trash receptacle inside women's toilet room. Instructed to provide.

- comments: observed heavy build up of grease, grime, dirt, and food debris on interior of fryer and on floor of cook line. Instructed to detail clean and maintain in good condition.

- comments: observed the following core violations not corrected from report #2200301 dated 7/30/2018:#37 ' all bulk food and spice containers must be labeled. ' #49 ' extremely dirty interior of the utility mop sink. Must clean and maintain. ' #56 ' insufficient lighting in the rear dry food storage closet. Must provide adequate lighting.
