- comments: observed approximately 300 mouse droppings scattered throughout basement storage rooms 1-4, (along walls and pallets, underneath boxes and items stored on floors)in staff lounge under water fountain area in wall, and in telephone room along walls. Instructed person in charge to remove all mouse droppings, sanitize all areas and inform pest control of activity. Priority foundation 7-38-020(a). Citation issued.

- comments: observed 1/4in gap at bottom of exterior door off kitchen area. Instructed person in charge to repair and maintain.

- comments: observed dead mouse along east wall in storage room 1. Observed dead mouse along north wall in storage room 3. Instructed person in charge to remove all dead mice and sanitize areas.

- comments: observed hand washing sink in dishroom to be slow draining. Instructed person in charge to repair and maintain.

- comments: observed excessive amounts of clutter stored on floors in storage rooms 1-4 in basement of facility (debris, trash, boxes, clothing, plastic bags, and maintenance items). Instructed person in charge to remove, organize, and maintain all items at least six inches from the floor at all times.
