- comments: no person in charge was present at the time of inspection. Priority foundation violation. No citation was issued.
2. City of chicago food service sanitation certificate
- comments: noted no certified food manager on duty while tcs foods (rice, chicken soups etc. ) were being prepared and served. Instructed management that a certified food manager with the city of chicago original food service certificate must be present at all times tcs foods are prepared and served, and also the person in charge must have a city of chicago certificate by next inspection (upon being inspected) priority foundation violation 7-38-012. Citation issued.
37. Food properly labeled; original container
- comments: noted assorted food items inside refrigeration units not properly labeled. Instructed to properly label all food items with product name, including bulk food containers.
38. Insects, rodents, & animals not present
- comments: noted over 10 rat droppings on floor along wall base at the of the basement food prep/dry storage. Instructed to detail, clean, and sanitize affected areas eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation violation #7-38-020(a). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: noted boxes of assorted food items stored underneath the waste pipelines at the basement food prep/storage area. Instructed to remove or relocate items from under pipes or encase pipes.
43. In-use utensils: properly stored
- comments: noted serving plates on shelves at the kitchen prep area not properly stored. Instructed to invert all serving plates to prevent dust accumulation.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted cardboard boxes used to line shelves at the basement food prep area. Instructed to clean and remove all cardboard boxes to prevent hiding places for pests.
55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean, sanitize and maintain floors throughout the kitchen prep areas underneath the cooking stove, equipment, at the basement prep/dry storage, and all of the dry storage area especially where the rat droppings were found.
56. Adequate ventilation & lighting; designated areas used
- comments: noted dirt and grease accumulation inside, on pipes/conduits, and on edges of the exhaust hood and filters above cooking equipment. Instructed to detail clean and maintain hood, filters, and pipes/conduits.
58. Allergen training as required
- comments: noted certified food manager on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification.
