- comments: observed no hand washing sign at hand washing sinks in bar and basement dish washing area. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: low temperature dish machine not sanitizing food equipment and utensils, chemical ppm 0 instructed manager to repair and maintain sanitizing rinse at 100 ppm. Dish machine is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-025 citation issued

- comments: observed 1/2' gap on bottom right corner of basement exit door. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed no chemical test available for dish machine. Instructed manager to provide test kit for proper concentration in sanitizing solutions and final rinse for dish machine. Priority foundation 7-38-005 citation issued

- comments: observed accumulated food debris and grease buildup on interior and exterior surfaces of cooking equipment, shelving units, prep tables, coolers, freezers. Instructed manager to clean and maintain.

- comments: observed grease buildup and debris on floor under all equipment, along walls and in all corners in prep, storage, and basement areas. Instructed manager to clean and maintain.
