- comments: no written employee health policy was provided on the premises. Management instructed to have a signed employee health policy for each employee. Priority foundation violation 7-38-010. Citation issued.

- comments: observed the pest control log book not maintained. No current pest inspection reports were observed in the provided pest control log book. Management instructed to provide 2 pest inspection reports from the last year and to keep the log book maintained at all times and available for inspection. Priority foundation violation 7-38-020(c). Citation issued.

- comments: observed evidence of rodent activity on the premises. Observed approx. 100 mouse droppings on the floor of the dry storage room and 2 non-working walk-in coolers that are used for storage under the dry storage shelves in the food prep area, around the grease trap under the 3-compartment sink, and in the small safe between the file cabinets and the ice machine. Management instructed to clean and remove all rodent droppings in all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.
