- comments: observed no hand washing signs at hand washing sinks in prep and washroom areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed over 20 mice droppings scattered on floor in office, prep, and basement areas, on ledge leading to basement and shelving units in basement. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued
