- comments: observed no city of chicago food sanitation manager on site while tcs foods are being prepped/served (such as chicken). Instructed must have a city of chicago food sanitation manager on site at all times when tcs foods are being prepped/served. Instructed must post a valid city of chicago food sanitation manager certification in plain view of customers at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no hand soap available at handsink in prep/diswashing area. Instructed to provide handsoap at all times. (corrected on site-manager provided soap) priority foundation violation 7-38-030 (c) citation issued

- comments: observed no paper towels at exposed hand washing sink in bathroom and dish area. Instructed to maintain paper towels at all exposed hand washing sinks at all times. Priority foundation violation 7-38-030(c). Citation combined with above violation.

- comments: must label food storage containers when food is not in original package.

- comments: observed no pest control logbook available at time of inspection. Instructed manager to provide. Logbook shall be available upon inspection. Priority foundation 7-38-020(c) citation issued

- comments: observed food items stored on floor in walk-in freezer. Instructed manager to elevate all food six inches off floor.

- comments: found wiping cloths stored improperly on prep tables throughout. Must store wiping cloths in sanitizer bucket between uses to prevent cross contamination. Instructed to maintain.

- comments: observed damaged/torn gaskets on reach in coolers in front prep area. Instructed manager to repair and maintain.

- comments: observed no chlorine or quats chemical test kit to measure the concentration of sanitizing solution. Must provide and maintain. (priority foundation 7-38-005)citation issued

- comments: must clean grease and food debris build up from fryer cabinets,wheel bases of equipment,grease filter,storage shelves,walk in cooler shelving units,window sills in lobby area,prep cooler and reach in freezer.

- comments: found excessive dried food debris in crevices of door gaskets on prep coolers in front prep area. Also, found dirt on exterior contact surfaces of large white food container stored under prep table. Instructed to detail clean and maintain.

- comments: observed slow drainage at the 3 comparment sink floor drainage and 1 compartment floor drainage. Instructed to repair or replace and maintain.

- comments: floor through out premises under and behind heavy equipment not clean, debris. Instd to clean and maintain.

- comments: observed employees in facility without food handlers certificates. Instructed manager that all employees must have training and must provide food handlers certificates.
