- comments: instructed to clean and maintain the following: interior bottoms of fryers to remove grease, interiors of refrigeration units, exterior surfaces of fryers to remove grease and food particles and bottoms of prep tables in prep areas and non prep areas. Also clean greasy equipment in rear storage area where droppings observed. Clean exterior of bulk storage containers.

- comments: observed the pipe connected to the exposed handsink in the front prep area is pvc piping; instructed to change to metal.
55. Physical facilities installed, maintained & clean
- comments: instructed to clean and maintain the following: floors under and around equipment where needed, detail clean floors in rear storage area where droppings observed to remove grease, ceiling vents with heavy dust and walls around, behind and above prep tables, freezers and other equipment. Instructed to clean the interior of the utility sink in rear storage area. Clean wall around utility sink.

- comments: observed five unused freezer in rear storage area; instructed to remove to avoid clutter. Also observed some unnecessary items in far rear storage area; instructed to remove to avoid clutter.
59. Previous priority foundation violation corrected
- comments: previous priority foundation violation not corrected from january 29, 2024 as described: observed rodent activity on premises. Found 300 mouse droppings on the floor and bottoms of prep tables in the far rear storage area where the rear exit is located, on the floor in the office area behind the desk and on top of the desk. Instructed to remove all droppings. Priority violation citation 7-42-090 issued.
