- comments: observed no paper towels located in the all gender washroom outside of the gym. Management instructed that paper towels must be provided at all hand washing sinks at all times. Priority foundation violation 7-38-030(c).
38. Insects, rodents, & animals not present
- comments: observed approx. 140 mouse droppings throughout the premises. Observed approx. 50 mouse droppings on the floor of the dry storage room, 20 mouse droppings in the food prep area on the edges of the mixer and the rolling cart used to support it, 20 droppings on the bottom shelf below the steam table and service line, 10 droppings on the floor of the cafeteria manager's office, 20 droppings in the unused second service area, and 20 droppings on the floor in the principal's office behind the back table and sides of the room. Management instructed to clean and remove all all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

- comments: observed no chemical test strips on the premises to measure the sanitizer concentration at the 3 compartment sink. Management instructed to provide. Priority foundation violation 7-38-005.
49. Non-food/food contact surfaces clean
- comments: clean the bottom shelf below the steam table.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in the all gender washroom outside of the gym. Management instructed to provide covered waste receptacles in all washrooms used by females.
