- comments: observed debris and grease build-up on cooking equipment, shelving units, and inside unused cold holding units in the kitchen/prep areas. Also, bulk storage containers need cleaning. Must detail clean and sanitize.

- comments: observed excessive clutter and unnecessary articles stored in several rooms throughout the facility. Must remove all unnecessary articles and/or organize. Remove and/or elevate items from the floors and maintain areas to prevent harborage.

- comments: must detail clean the wall, ceiling, and attachment(pipe, vent cover) to remove grease, dust, and debris.
