38. Insects, rodents, & animals not present
- comments: observed approx 150-200 live roaches throughout facility. Seen in the floor drain left of the hand washing sink. Seen under thr prep table near the three comp sink upon entrance of the food
prep area. Roaches seen crawling along base boards behind three compartment sink in rear food prep area, also noticed roaches in rear dry storage area crawling inside oil containers and along base of wall under shelving units. Roaches observed crawling on pipes behind and inside wall behind the mop sink. Roaches seen crawling behind and inside unused equipment in front food service area. Also roaches crawling under and around equipment in the front service area where the cash register is located. Noted roaches crawling on walls, on and underneath all equipment, crawling in and out of openings in walls and wallbases throughout premises. Also observed over 25-30 large and small flies on walls and floors and on equipment. Instructed to have pest control service for roaches and flies with report of service before return inspection date. Priority citation issued 7-38-020(a)

- comments: observed a heavy build up of dirt, grease and food debris under and around all shelving units, refrigeration units,cooking equipment, sinks and all other heavy equipment throughout. Instructed to clean, sanitize and maintain.

- comments: observed excessive build up of dirt,grease and food debris on the floors under shelving units, under cooking equipment, under sinks,under all other heavy equipment and in washroom and storage areas. Also detail clean excessive food debris on floors inside walk in cooler and freezer. Instructed to clean, sanitize and maintain.
49. Non-food/food contact surfaces clean
- comments: noted build up of dust and slimy substances on interior of ice machine. Detail clean the interior of the ice machine including the lining and access door. Instructed to clean, sanitize and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed only one toilet room in facility with more than ten employees on duty at the time of the inspection. Instructed to provide additional toilet room to and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observerd outside garbage area overflowing with trash, unable to close lids. Noted multiple buckets of waste grease and soiled linen stored outside next to the garbage area uncovered and attracting insects. Instructed to remove all excessive garbage,soiled linen and waste grease. Priority foundation 7-38-020(b) issued

- comments: observed floors in rear and front food service area with a build up of dirt and food debris. Instructed to clean, sanitize and maintain.

- comments: observed openings in the wall coving along the base line of the floors in the rear and front food service areas near the three compartment sink, near the rear hand washing sink where the floor drain is. Along the piping area of the rear mop sink and the hot water unit. Instructed to repair and maintain.

- comments: observed clutter in all interior storage areas. Also noted excessive clutter throughout backyard storage areas. Instructed to remove all unnecessary items,clutter, unused equipment. Must clean, and maintain.

- comments: observed vents and fixtures inside walk in coolers and throughout facility with a heavy build up of dust. Instructed to clean, sanitize and maintain.
