- comments: 4-501. 11 observed rusted racks in walk in cooler. Instructed manager to replace racks with a corrosion resistent, non-absorbent, easily cleanable material.

- comments: 4-501. 12 observed deep dark scoring on cutting prep chicken board. Instructed that cutting board must be resurfaced or replaced.
54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 115 observed excessive accumulated clutter located by the outside dumpster area in the rear. Instructed to remove all unnecessary clutter.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 114 observed clutter and unnecessary items in dry storage, in rear near exit doors and in rear closet storage area. Instructed manager to remove unnecessary items and elevate necessary items six inches off floor.

- comments: 4-202. 18 observed excessive amount of grease accumulation on filters above griddle. Instructed manager to clean observed heavy dust build up in vents above food prep area. Instructed manager to clean dust in vents to avoid contamination.

- comments: 8-404. 13 (b:3) observed previous priority foundation violation not corrected from report #2554708 dated on april 22nd 2022. Observed approximately 60 rodent droppings in office areas; approximately 15 rodent droppings near dumpster door; about 5 rodent droppings in utility room, and approximately 5 rodent droppings in dry storage area. Instructed manager to clean and sanitize all surfaces where mice droppings are present. Priority violtion 7-42-090. Citation issued.
