- comments: observed mold on lemon cake displayed for sale in display case. Instructed manager to discard. All food displayed for sale shall be safe and in good condtion. Priority 7-38-005 citation issued

21. Proper hot holding temperatures
- comments: observed tcs food stored at improper temperature on prep table, chicken 83. 8f, beef 96. 3f, egg rolls 101. 4f. Instructed manager to discard. All hot food must maintain 135f or above. Priority 7-38-005 citation issued.

22. Proper cold holding temperatures
- comments: observed cheese stored at improper temperature 53. 2f on prep table. Instructed manager to discard. All cold food must maintain 41f or below. Priority 7-38-005

23. Proper date marking and disposition
- comments: observed ready to eat tcs food(cheese, sour cream, salads, pasta,etc) not labeled and date marked. Ready to eat tcs food prepared and held in retail food establishment more than 24 hours shall be clearly marked with sell by date which food shall be consumed, sold or discarded for a maximum of 7 days. Priority foundation 7-38-005 citation issued

- comments: observed bulk containers not properly labeled in prep and storage areas. Instructed manager to label all bulk containers with common name of food.

38. Insects, rodents, & animals not present
- comments: observed three live roaches, several flying insects and over 50 mice droppings scattered on floor along walls and corners in prep and storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed spoon used as dispensing utensil inside bag of ice. Instructed manager to provide proper ice scoop for dispensing ice.

- comments: observed multi-use equipment stored on floor in prep and storage areas. Instructed manager to clean and maintain.

- comments: observed single service articles stored on floor and not inverted in prep and storage areas. Instructed manager to elevate and invert all single service articles to protect from contamination.

- comments: observed worn cutting boards with deep grooves in use in prep area. Instructed manager to replace and maintain.

- comments: observed food debris and grease buildup on interior and exterior surfaces of cooking equipment, coolers, freezers, bulk containers, shelving units, ledges, prep tables. Instructed manager to clean and maintain.

- comments: observed leaking sprayer on 3-compartment sink. Instructed manager to repair and maintain.

- comments: observed outside garbage dumpster not covered, grease and debris spills on groud around dumpter. Instructed manager to cover all dumpsters and maintain outside garbage area. Priority foundation 7-38-030(b) citation issued.

- comments: observed heavy dust, dirt, debris and buildup on walls and floors under all equipment, along walls and in all corners in prep, storage and washroom areas. Observed dirt and stains on walls and doors in prep, washroom, and office areas. Instructed manager to clean and maintain.

- comments: observed the ventilation hood above the grill with severe dust/ charcoal build up on the entire ventilation hood. Instructed to clean the ventilation hood more frequently and maintain.
