- comments: 6-301. 14: observed no handwashing sign at the handwashing sink behind the bar. Instructed to provide and maintain.

- comments: 6-501. 111: observed approximately 80 or more mice droppings throughout the facility. Mice droppings were observed in the following areas: on the floor in the mop sink closet, floor sounding the dishwashing/prep area, on the floor in the ice machine storage area, on the floors surrounding cooking equipment and prep tables, on the floor in the storage room near the rear exit door, on two cutting boards stored on the lower shelf of clean dishware, on the shelf under the food prep table. Instructed to remove mice droppings, clean, and sanitize all affected areas. Recommend contacting pest control. Priority foundation violation 7-38-020(a), citation issued

- comments: 4-601. 11(b): observed the lids on the black and yellow bulk food storage containers covered in sauce and not sanitary. Also, observed black and pink build up on the ice machine. Instructed to wash, rinse, and sanitize mentioned surfaces.

- comments: 6-201. 12: observed an opening in the lower wall behind the ice machine. Instructed to seal to eliminate possible pest entry/ harborage points.

Instructed to paint or replace as a means of preventing possible contamination. #55: observed the floor in the food service area with a build up of dirt and debris along the walls under the counters and other non cooking equipment. Instructed to clean and maintain all areas. Instructed to corrected all violations. Priority foundation:7-42-090, citation issued
