- comments: observed food items stored on the floor in deli walk-in cooler, located in the rear receiving area. Instructed the person in charge to elevate all food six inches off the floor.

- comments: observed an excessive amount of debris and ice buildup on the floor/walls/ceiling of all walk-in freezers. Instructed the person in charge to remove excess ice, detail clean, and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed loose debris, dirt, and dust buildup on the floors and corners throughout the facility. Instructed the person in charge to detail cleaning and increase the cleaning frequency of floors. Must maintain at all times.
56. Adequate ventilation & lighting; designated areas used
- comments: observed a missing light shield for the light above the handsink in the women's toilet room, located on the second floor. Instructed the person in charge to provide and maintain at all times.
