- comments: observed cross contamination of cooked eggrolls. The eggrolls were stored in the prep cooler inside a cardboard box previously used to store raw chicken. Management instructed to store all foods in a proper container so as not to cross-contaminate the food. Priority violation 7-38-005. Citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory reminder statement on the menu to notify the customers that consuming raw or undercooked foods can lead to an increased risk of foodborne illness. Management instructed to provide the consumer advisory reminder statement on the menu after the disclosure that 'rolls contain raw sashimi. ' priority foundation violation 7-38-005. No citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed an electric bug zapper stored in the food prep area. Management instructed to remove from the food prep area.

- comments: replace the torn door gasket inside of the 2 door prep cooler next to the cook's line.

- comments: clean the grease and food debris inside of the prep coolers, on the outside doors and handles of the basement freezers, rolling prep cart at the cook's line used for spices, deep fryer, stove and broiler in the prep area.

- comments: remove excessive clutter and unused items from the basement storage rooms. Store all remaining items at least 6' off of the floor to eliminate pest harborage conditions.

- comments: clean the excessive grease and soot from the ventilation hood and filters above the cooking equipment.

- comments: previous core violations from report #2580631 on 8/24/23 not corrected: 53 - observed no covered trash receptacle inside the women's toilet room and employee toilet room. Instructed to provide. 57 - observed no food handler training certificates on site at the inspection. Instructed all employees must complete food handler training and maintain records thereof on site at all times. Priority foundation violation 7-42-090. Citation issued.

62. Compliance with clean indoor air ordinance
- comments: observed a violation of the clean indoor air ordinance. Observed a container used as an ashtray in the basement employee washroom with ashes in the container and ashes and cigarette butts on the floor of the washroom. Management instructed that no smoking is allowed inside of the establishment and that ashtrays cannot be stored inside the establishment. Clean indoor air ordinance 7-32-020(c). Citation issued.
