- comments: found broken gaskets on the 2-door true cooler in the front prep area. Must repair or replace torn gaskets and maintain.

- comments: found old onion debris on the exterior of the onion storage containers. Must clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed a heavy build-up of grease and dirt inside the crevices of the floor mats in the front prep area. Found dirt build-up in the window seals at the front service counter. Must detail clean said areas and maintain.

58. Allergen training as required
- comments: observed multiple city of chicago food managers without allergen training. Instructed that all city of chicago food managers must complete allergen training and keep certificate on site.
