- comments: the person in charge during the inspection does not have a city of chicago sanitation certificate. Priority foundation violation.
2. City of chicago food service sanitation certificate
- comments: no valid city of chicago sanitation certificate was posted on the premises during the inspection. A manager arrived with an expired sanitation certificate in hand during the inspection 45 minutes after being telephoned by an employee on site. Reviewed a city of chicago certified food manager must be on-site at all times foods are being prepared and served. Priority foundation violation 7-38-012 citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no written employee health policy on-site and available for review during the inspection. Must provide for all employees. Priority foundation violation.
5. Procedures for responding to vomiting and diarrheal events
- comments: no written cleaning procedure or required equipment on site for a vomit/diarrhea event. Must provide for all inspections. Priority foundation violation.
10. Adequate handwashing sinks properly supplied and accessible
- comments: no paper towels available at the women's washroom hand washing sink used for customers and employees. Must provide paper towels at all times. Priority foundation violation 7-38-030(c) citation issued.

- comments: observed prepared tcs foods such as tuna salad, sliced deli meats, and cheeses not properly date labeled with an expiration/use by date. Prepared foods noted with a prep date, shift time, and food handler initials. No written procedure or policy for identifying expired foods or a use-by date. Foods discarded. Reviewed requirements for date labeling. Priority foundation violation 7-38-005 citation issued.

- comments: all bulk spice containers must be labeled with the product common name.

- comments: must not store spray water bottles used to spray bread dough on the rim of the handwashing sink. Instructed to store in a clean area/prep surface. ---noted the plastic ice scoop with a broken handle repaired with tape. Must replace ice scoop.

- comments: all stored items in the dining room closet must be elevated and organized from the floor.

- comments: extremely large ice pile build up on the floor of the walk-in freezer. Instructed to remove.

- comments: all unnecessary items throughout the basement must be removed from the premises, and all other need items must elevate and organized.

- comments: the previous core violation from report #2382684 on 7-8-20 was not corrected. #49- 4-601. 11(c)-- observed food debris/liquid build up inside upper portion of backup 3-door prep reach-in cooler. Must clean interior of cooler and maintain. #16- 4-602. 11-- observed black build-up inside ice machine. Must clean interior of ice machine and maintain. Priority foundation violation.
