- comments: instructed to provide thermometer visible inside all prep coolers and in basement walk-in coolers.

37. Food properly labeled; original container
- comments: instructed to properly label (faded) food storage containers identified with common (flour, sugar etc) in the kitchen and in basement prep area.

38. Insects, rodents, & animals not present
- comments: observed approximately over 25 small live flies on walls and ceiling in basement prep area. Instructed to remove, clean and sanitize the affected areas. Recommend to contact licensed pest control for service. Priority foundation violation 7-38-020 (a) citation issued

48. Warewashing facilities: installed, maintained & used; test strips
- comments: found no irreversible registering temperature indicators or test strips/stickers were provided to measure the surface temperature of utensils/plates (high temperature dishmachine with gauge reading 185f). Instructed manager to provide surface temperature test strips or irreversible (surface) registering thermometer to take the temperature of the water inside high temperature dishmachine. Priority foundation 7-38-005

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed leaves and debris on ground under and around garbage dumpster in rear outside area enclosure. Instructed to clean ground and maintain.
