- comments: found an employee in the food prep area without wearing a hair restraint. Instructed manager to have all employees entering the food prep area to wear a hair restraint. Maintain same.

41. Wiping cloths: properly used & stored
- comments: found soiled wiping cloths stored on prep tables. Instructed manager to store wiping cloths in sanitizing solution (must be at the required ppm) between uses.

55. Physical facilities installed, maintained & clean
- comments: found the floors under/around the heavy equipment and in all corners with a build up of dirt, grease, food and dust. Instructed manager to clean and maintain floors throughout.

- comments: all food handers that do not have the city of chicago food sanitation certificate must meet the food handlers requirement and provide proof. Maintain same.
