- comments: observed no backflow prevention device on the water line to ice machine, utility sink, and coffee machines. Instructed to provide backflow prevention devices that can serviced and easily located.

Instructed to provide hand washing signage at all hand washing sinks. #53-obsereved no covered trash receptacle in the toilet room. Instructed to provide a covered trash receptacle in toilet room and maintain. #55-observed dirt on floors and under shelves throughout the establishment. Instructed to detail clean and maintain. #55 - observed clutter in rear storage area. Must remove unnecessary articles and clutter.
