- comments: observed no consumer advisory disclosure nor reminder on menu board at window of consuming raw and undercooked foods. Must provide and maintain. (priority foundation 7-38-005)

- comments: must label food storage contaniers (shakers) when food is not in original package.

38. Insects, rodents, & animals not present
- comments: observed evidence of 6- live roaches crawling on wall at hand sink,on floor amongst excessive grease and food debris build up under and behind cooking equipment. Appx. 75 or more mice droppings on floor under food storage shelf in rear storage area and on all shelving units at service counter. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 8/8/24. (priority foundation 7-38-020(a)(citation issued)

41. Wiping cloths: properly used & stored
- comments: must store wiping cloths in a sanatizing solution when not in use from surfaces.

- comments: must remove cardboard from overhead shelving unit& lower shelf of prep table in front prep area. Must remove rust from lower shelf of prep table in rear prep area & lid of chest freezer. Must repair or replace damaged contact paper on shelving units at service counter to be smooth and easily cleanable.

- comments: must repair or replace damaged lid & door gaskets on chest freezer and worn gaskets on 2- door reach in cooler in rear area.

- comments: must clean excessive, grease and food debris build up from interior and exterior of fryer cabinets, grill table,prep tables,unused coolers,pipes of cooking equipment,legs & wheel bases of equipment,mopsink basin,3- compartment sink basins,undersides of all equipment,tables & service counter in dining area,unused ice machine. Must defrost ice build up from chest freezer.

- comments: must repair or replace leaky pipe at hand sink in prep area,leaky pipe and faucet at 3- compartment sink.

- comments: must repair or replace missing wall tiles behind dish storage rack,loose wood panel on ceiling above rear reach in cooler,damaged wall behind and under 3- comp sink,damaged floor tiles in dining area. Must seal all gaps and cracks in walls ,service counter and where needed.

Must replace and maintain to prevent entry point for pests. #49-4-601. 11(c) : must clean and maintain dry food storage shelving. Must remove debris. #55-6-501. 12 : observed the floors through out encrusted with spills, debris and/or grease. Must detail clean and maintain. #55-6-201. 17 : observed the light shield, air vent cover dirty through out. Must clean and maintain#55-. 6-201. 16 : observed the celing in the front prep area in poor repair(missing and/or damaged). Must replace and maintain. (priority foundation 7-42-090)(citation issued)
