- comments: 3-door pizza prep cooler door rubber gaskets ripped/torn. Insructed manager must repair/replace damaged door rubber gaskets inside said cooler.

- comments: observed food debris inside bar gun holder and food/dust debris build up on ventilation hood. Must clean and maintain equipment.

- comments: observed one certified food manager without food allergen training certificate. Instructed all city of chicago certified food managers must have food allergen training certificates.
