- comments: observed unused equipment on site. Instructed to remove to prevent harborage for pest.

- comments: observed previous core violation #58 not corrected from report #2498053 conducted on 4/12/21: - observed no allergen training certificates on site for the city of chicago certified food manager. Instructed to meet requirements. Must provide completed certificates for all persons with city of chicago cfms. #25 observed no consumer advisory on the menu (electronic and printed) for all animal food products that can be ordered raw or undercooked. Instructed to provide a consumer advisory on said menus, including disclosure and reminder(s) for all animal-derived food items (beef burgers and eggs any style) that can be ordered raw or undercooked.
