Instructed to meet requirements. Must provide completed certificates for all persons with city of chicago cfms. #25 observed no consumer advisory on the menu (electronic and printed) for all animal food products that can be ordered raw or undercooked. Instructed to provide a consumer advisory on said menus, including disclosure and reminder(s) for all animal-derived food items (beef burgers and eggs any style) that can be ordered raw or undercooked.
