- comments: found a fly/pest light device directly over an in use prep table. Instructed management to relocate pest device or relocate prep table.

- comments: found boxes of food throughout the facility stored directly on the floor (in the basement walk-in cooler, the outdoor walk-in cooler, and in the dry storage area). Must elevate food off floor.
45. Single-use/single-service articles: properly stored & used
- comments: found unwrapped clean single service spoons throughout the facility stored with the handles facing down. Instructed to invert spoons to protect from miscellaneous sources of contamination and so that employees only touch spoons by handles.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: found a missing drain hose for the soda gun holder at the 1st floor bar. Must provide.
