- comments: 3-305. 12 observed shelves for food (bags of raw onions) and equipment (pots and pans etc) stored next to and under a sewer line. Instructed to moves shelves and do not store foods or equipment near or under sewer lines.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-501. 17 observed no covered receptacle inside of the women's employee toilet room. Instructed to provide.

54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 15 observed outside dumpster overflowing with garbage bags. Instructed to never allow outside dumpster to overflow and maintain lids tight fitting at all times. Priority foundation 7-38-020(b).

55. Physical facilities installed, maintained & clean
- comments: 6-201. 13 observed coving in poor repair and deattached from the wall under the prep table that is located across from the 3 compartment sink. Instructed to repair.
