- comments: observed packaged desserts not properly labeled. Management instructed to label all prepackaged food with name and address of manufacturer, and net weight or contents.

38. Insects, rodents, & animals not present
- comments: clean and remove all dead insects from the floor under the storage shelves and next to the freezers.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: remove the aluminum foil stored on the metal storage shelf next to the oven.

- comments: observed noodles stored in direct contact with a cardboard box underneath the prep table. Management instructed to use only food grade containers or storage bags.

49. Non-food/food contact surfaces clean
- comments: detail clean the grease and food debris on all cooking equipment such as the oven, grill, deep fryers, prep tables, wire shelves in prep coolers, microwave, toaster oven. Clean the heavy grease buildup on the gas lines behind the cooking equipment. Clean the interior surfaces of the ice machine. Defrost and maintain 1 door freezer of excessive ice and frost buildup inside.

50. Hot & cold water available; adequate pressure
- comments: observed no hot water provided on the premises while tcs foods were being prepared. The hot water at the 3 compartment sink was at 65f. Management instructed to provide adequate hot water above 110f at the 3 compartment sink and above 100f at all handwashing sinks. Priority 7-38-030(c). Citation issued.

52. Sewage & waste water properly disposed
- comments: observed waste water overflowing from the open drain at the grease trap under the 3 compartment sink. Management instructed to repair and maintain the grease trap and drain so that the water drains without overflowing. Must remove the rusted section at the bottom of the grease trap and maintain. Priority 7-38-030(c). Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in the women's washroom. Management instructed to provide.

55. Physical facilities installed, maintained & clean
- comments: observed cardboard flats used in the food prep area as floor mats. Management instructed to remove the cardboard and use a cleanable material as a floor mat.

- comments: clean the grease and food debris on the wall next to and behind the cooking equipment. Clean the grease on the floor under the cooking equipment. Clean the food debris on the floor next to the freezers. Clean the dust on the ceiling tiles and light shields in the food prep area.

- comments: clean the grease buildup on the ventilation hood and filters.
