- comments: observed approx. 50 live small flying insects on the walls, ceiling and boxes of stock in the food prep area, mop area, manager's office and one of the washrooms. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: clean the prep tables and hot holding units above the service island.

- comments: clean the walls of the food prep area. Clean the stained ceiling tiles in the food prep area.
