Report date: Apr 21, 202310. Adequate handwashing sinks properly supplied and accessible
- comments: observed hot water at 132f at the front prep handwashing sink. No cold water was provided to the exposed handwashing sink. Must provide hot water between 100-120f at the handwashing sinks. Priority foundation 7-38-030(c). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the walk-in cooler. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: torn gasket at bottom of walk in cooler door. Instd to repair and maintain

- comments: the following not clean excessive grease/debris: sides of cooking equipment, legs & wheels of all tables, storage shelves, fryers, exterior of syrup boxes in rear and interior of cabinet under beverage station. Instd to clean and maintain

- comments: excessive leaking and loose cold water faucet handle at 3-compartment sink. Instd to repair and maintain

- comments: missing grout and worn grout lines around floor tiles through out entire prep area and warewashing area causing excessive debris accumulation. Instd to re grout and maintain floor so as to be as smooth, even, easily cleanable surface. Outlet covers missing around outlets in prep area. Instd to provide covers and maintain. Excessive foam being used around holes and piping in floor, walls, and ceiling. Instd to remove foam and use proper materials so as surface to be smooth, even and easily cleanable. Peeiling surface on ceiling tiles through out premises. Instd to repair/replace ceiling tiles and maintain.

- comments: lightshields missing in rear by 3-compartment sink. Instd to provide and maintain
60. Previous core violation corrected
- comments: previous core violations from 7/15/2022 report 2560832 not corrected: 53 5-501. 17: **c** toilet room receptacles, covered no cover on waste receptacle in womens washroom; must provide a waste receptacle with a cover. 55 6-501. 12: **c** cleaning, frequency and restrictions instructed to clean floors and walls around, behind and under all kitchen equipment (cooking, sinks, refrigeration units, hot holding units, prep tables, closet. 55 6-501. 11: **c** maintained in good repair, facilities (facilities not in good repair) observed the interior door of the walk-in cooler rusted; instructed to replace or paint. Observed other metal panel around the walk in freezer door in poor repair; instructed to repair. Citation issued 7-42-090 priority foundation.
