- comments: unable to locate thermometer inside of 3-door cooling unit located in grocery area. Instructed to provide.
38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on site during the inspection, noted approximately 60 small rodent droppings in the following places. . . In the rear storage/prep area on lower-level shelving units, on top of boxes, on top of unused equipment (toaster) scattered on the floor near black stacked milk crates, in the corner next to in place glue traps by the front window in the grocery area, on floors in corner and shelving units in rear storage room behind reach in coolers. Instructed to clean and sanitize all said areas. Must eliminate activity.

- comments: observed display cooler in front prep area with accululated food debris on interior and exterior surfaces. Instructed to clean and maintain free of accumulation.

- comments: observed receptacle inside of unisex toilet room without covering. Instructed to provide.

- comments: observed water-stained ceiling tiles in the storage room attached to the rear prep area. Instructed to replace.
