- comments: black slime substance at interior of ice machine dripping onto ice cubes at ice bin. Ice is used for human consumption. Instructed to clean, sanitize and maintain unit. Priority foundation violation:7-38-005, citation issued

41. Wiping cloths: properly used & stored
- comments: must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross-contamination. Instructed to maintain.

43. In-use utensils: properly stored
- comments: observed knives being stored between towels at the front prep counter. Must provide a proper knife holder to minimize source of contamination

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored items.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: noted no irreversible registering temperature indicators for high-temperature dish machine. Instructed to provide an irreversible registering temperature indicator, such as strips/stickers or thermometer. Priority foundation violation: 7-38-005. No citation was issued.

49. Non-food/food contact surfaces clean
- comments: rubber gasket inside the reach-in cooler and refrigerator with dry debris,clean sanitize, and maintain

- comments: leaking faucet at three compartment sink, also leaking backflow prevention device at high temp dish machine, repair issues
