3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)(citation issued)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs at hand sinks in middle and basement prep areas. Must provide and maintain.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 100 or more mice droppings inside of fryer cabinet,lower shelf of rolling cart,along wall base ledges,back of refrigerators and stove,service counter shelving units, furnance room. Also, evidence of 1-live roach crawling on floor in dining area and appx. 7-9 live small flies flying around plants and on walls in dining area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/11/22. (priority foundation 7-38-020(a)(citation issued)

39. Contamination prevented during food preparation, storage & display
- comments: must provide a splash guard between mop sink and drainboard at 3- compartment sink.

- comments: must eliminate crates used for food storage in dry food storage area and provide adequate shelving units.

- comments: must defrost ice build up in chest freezer. Must clean grease and food debris build up from grill table,upper shelving units,window sills in dining area.

- comments: must repair or replace damaged floor tiles on staircase to basement,at front entry door,rear dry food storage area concrete floor in poor repair. Must grout floor in dish washing area and where needed.
