16. Food-contact surfaces: cleaned & sanitized
- comments: 4-602. 11(b-e) observed a minor slight brownish substance located on the panel of the ice machine located in the rear food prep area. Instructed to clean and sanitize.

25. Consumer advisory provided for raw/undercooked food
- comments: 3-603. 11 observed no consumer advisory disclosures on the breakfast items side of the menu for the physical menus and the online menu. Observed no consumer advisory reminder on the breakfast online menu. Instructed to provide an astreik on the breakfast egg items that can be ordered undercooked or raw and add an astreik next to the consumer advisory reminder that is presently on the physical menu and add the consumer advisory disclosure and reminder on the online breakfast menu. Priority foundation 7-38-005.

41. Wiping cloths: properly used & stored
- comments: 3-304. 14(b) observed in the use of wiping cloths for wiping equipment not stored in a sanitizing solution. Instructed: all wiping cloths for equipment must be stored in a sanitizing solution and not stored on top of the cutting boards.

43. In-use utensils: properly stored
- comments: 3-304. 12 observed an ice scoop being stored directly in an ice bin without the handle above the ice bin container. Instructed to store the scoop handle upright to avoid possible contamination of ice.

- comments: 4-502. 13 observed single-use containers being re-used for food storage. Observed single-use cardboard being re-used as a floor liner. Instructed to not reuse single-use articles.

- comments: 4-601. 11(c) observed excessive food and grease debris under the flattop grill as well as inside the fryer unit. Instructed to remove all debris, clean and sanitize.

- comments: 5-204. 12 unable to locate the backflow prevention device on the ice machine located in the rear. Instructed to locate or provide.

- comments: 6-501. 114 observed items (bag of onion, bag of beans) stored directly on the floor in the rear food prep area. Instructed to elevate everything at least 6 inches from the floor and maintain.

- comments: 7-38-060 observed a kid's menu on site without an adequate default beverage. Children's meal default beverage should include one of the following. (1) water with no added natural or artificial sweeteners; (2) sparkling water with no added natural or artificial sweeteners; (3) flavored water with no added natural or artificial sweeteners; (4) nonfat or 1% dairy milk containing no more than 130 calories per container or serving offered for sale; (5) nondairy milk alternatives: (a) with no added natural or artificial sweeteners; (b) containing no more than 130 calories per container or serving as offered for sale; and (c) meeting the standards for the national school lunch program as set forth in 7cfr210. 10; or (6) 100% fruit or vegetable juice or juice combined with water or carbonated water, with no added sweeteners, in a serving size of no more than 8 ounces. Instructed to provide.
