3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on the premises. Management instructed to provide and maintain a signed employee health policy for each employee. Priority foundation violation 7-38-010,citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: procedure/plan for responding to vomiting and diarrheal events on site but has in-complete clean-up items/kit. Instructed to provide and maintain appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer that is effective against norovirus). Priority foundation violation 7-38-005,no citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: instructed to provide and maintain hand washing signage at hand sink for front prep area.

36. Thermometers provided & accurate
- comments: observed no metal stem thermometer for taking food temperatures. Must provide and maintain. Priority foundation violation 7-38-005,no citation issued.

40. Personal cleanliness
- comments: observed food handling employee prepare food without wearing a hair restraint. Management instructed that all food handling employees must wear a hair restraint.

41. Wiping cloths: properly used & stored
- comments: instructed to properly store and maintain all washcloths in a sanitizing solution especially when not in use.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed stained cutting boards in the food prep areas. Management instructed to resurface or replace the cutting boards.

- comments: observed raw wood shelves in the rear storage area. Management instructed to paint or seal the shelves.

- comments: observed use of milk crates for elevation at the dry storage area. Instructed not to use milk crates for elevation due to hiding places for pests. Must provide appropriate shelving for storage. All food items must also be stored 6' away from the wall.

- comments: observed small water leak at water line underneath the dish washing 3-compartment sink. Must repair plumbing leak and to maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: a toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

55. Physical facilities installed, maintained & clean
- comments: instructed to replace water stained ceiling tiles at rear dry storage area and above the rear mop sink area.

57. All food employees have food handler training
- comments: food handler with no proof of food handler training. Instructed to provide. Information given.

58. Allergen training as required
- comments: observed certified food manager on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification.

61. Summary report displayed and visible to the public
- comments: previous inspection report summary from (08/02/2018) was not displayed for the public to view. Today's inspection report summary was issued and must be displayed and visible to the public at all times. Priority foundation violation 7-42-010 (b), citation issued.

64. Public health nuisance
- comments: observed person in charge behind the front counter not wearing a face covering at this time. Management instructed that all employees must wear a face mask or face covering over their nose and mouth while working. Citation issued 7-28-060. - (cos=corrected on site during inspection).
