- comments: noted 1/2 inch gap on right side of the exit door. Must rodent proof said door and make tight fitting.

- comments: noted garbage bins being used for food storage containers. Must store all food in approved food grade containers.
49. Non-food/food contact surfaces clean
- comments: observed multiple soiled chest freezer gaskets. Must detail clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed unnecessary items stored throughout the back storage and prep areas. Instructed management to remove any unnecessary articles from premises, reorganize remaining articles, and store items off floor to allow for facility cleaning.
59. Previous priority foundation violation corrected
- comments: noted previous priority foudation violation not corrected from report #2556847 on 6/6/22: violation #38 (6-501. 111) observed over 20 live cockroaches in the following areas: inside equipment in the dry storage room, between sheet metal shelf on clean side three compartment sink, inside bean storage container, on door between kitchen and prep area, underneath the storage table top located next to exit door, and within wall and fire suppression release valve. Must provide pest control receipt stating no activity. Instructed management to correct previous priority foundation violations. Priority violation 7-42-090.
