- comments: no pest control log book with necessary documents as required on premises from new company servicing establishment. Instructed to provide and maintain. Priority foundation violation 7-38-020(c) citation issued.

- comments: observed rodent infestation on premises. Found 300 or more mouse droppings on the floor in the walk in cooler, on the floor under sinks, cooking equipment, storage areas, pit area, inside of unused pit in pit area, rear storage area, closets, inside the walk-in cooler on the floor and on window sills in front storage area. Instructed to clean all areas and have pest control company service. Priority violation 7-38-020(a) citation issued.

- comments: instructed to clean the exterior surfaces of all cooking equipment, bottoms of fryers, interiors of hot holding units and refrigeration units including the walk-in cooler shelves.

- comments: hot running water not getting above 93f at any sink on premises. Instructed that hot running water must be maintained at a minimum of 100f at all handsinks and at a minimum of 110f at the three comp sink. Must correct and maintain. Priority violation 7-38-030(c) citation issued. Hot running water restored to 100f and 112f. Corrected on site.

- comments: must clean and maintain the following: floors, walls and ceiling tiles where needed throughout kitchen and storage areas. Clean the vent fan cover in the washroom on the ceiling to remove dust.
