2. City of chicago food service sanitation certificate
- comments: observed no city of chicago food sanitation manager on site while tcs foods are being prepped/served (such as chicken). Instructed must have a city of chicago food sanitation manager on site at all times when tcs foods are being prepped/served. Instructed must post a valid city of chicago food sanitation manager certification in plain view of customers at all times. Priority foundation violation 7-38-012. Citation issued.

22. Proper cold holding temperatures
- comments: observed tcs foods at improper temperatures. Found pasta at 49. 8f, rice at 48. 9f and chicken at 48. 6f in walk-in cooler. Instructed all tcs foods must be held at 41f or less at all times. Operator voluntarily discarded denatured foods during inspection. Priority violationi 7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed walk-in cooler not maintaining proper temperature 49. 2f. Instructed manager to repair and maintain cooler at 41f or below. Prep cooler is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued.

38. Insects, rodents, & animals not present
- comments: observed no current pest control logbook for 2023. Must provide contract, logbook and recent pest control service. Priority foundation 7-38-020(c)

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chlorine or quats sanitizer testing devices on site at time of inspection. Instructed to provide. Priority foundation violation 7-38-005. Citation issued

49. Non-food/food contact surfaces clean
- comments: observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units. Instructed manager to clean and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed debris around dumpster. Instructed to clean and maintain

55. Physical facilities installed, maintained & clean
- comments: observed a heavy accumulation of dirt, dust, grease, food splatter and food debris on the walls and floors especially along the hot food service line under and around the cooking equipment and non cooking equipment. Instructed to clean and maintain all areas.

56. Adequate ventilation & lighting; designated areas used
- comments: observed the hood ventilation filters with a build up of dust and grease also observed the pipes and fire system. Instructed to clean and maintain as a means of helping prevent possible contamination of foods being prepared as it sits over a cold prep station.

57. All food employees have food handler training
- comments: observed no food handler certificates on site at time of inspection. Instructed all employees must complete food handler training and maintain records thereof on site at all times.

58. Allergen training as required
- comments: observed no allergen training on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificates must complete allergen training and maintain records thereof on site at all times.
