- comments: observed facility food handlers preparing and handling time and temperature control for safety(tcs) sauce, chicken, diced ham) without original city of chicago food service manager and certificate on site. Informed person in charge of city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: oberved no papertowel available at handsinks located in both in-use prep areas. Instructed to provide and maintain proper handwashing. Priority foundation violation. #7-38-030 (c). Citation issued.

- comments: no thermometers inside cooling units throughout the prep area. Instructed to provide.

- comments: observed no metal stem thermometer on site for testing internal temperatures of food. Insructed to provide and maintain on-site for use.

- comments: observed 50 small live flies flying throughout the rear dishwashing/prep area as well as rear storage space on cardboard boxes, flying around in the toilet room near the rear exit door. Noted approximately 35 small rodent droppings in the rear prep/dishwashing area scattered along the wall under rolling carts, in front prep pizza boxing and slicing prep. Instructed to contact pest control for service, remove droppings, clean and sanitize area. Must eliminate fly activity. Priority foundation violation. 7-38-020 (a). Citation issued.
49. Non-food/food contact surfaces clean
- comments: observed black mold build up on wall behind 3 compartment sink, excessive sauce splatter on cooling units, wunder bar sauce spinner, all shelving units and elvation units containing flour in rear prep area. Instructed to clean.

- comments: observed front prep handsink slow draining. Instructed to repair and maintain in good condition.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed both toilet room plumbing fixtures in need of cleaning and instructed to provide and maintain.

- comments: observed wall in front pizza prep, all rolling carts and leaning shelving unit inside walk in cooler with an immoderate amount of sauce and dried food splatter. Instructed to clean in order to prevent pest harborage.

- comments: observed wall crumbled and in disrepair with large gap between the wal and floor base underneath handsink located in rear prep/dishwashing area. Instructed to replace all missing wall base throughout rear prep area and maintain.

- comments: observed floors throughout front and rear prep areas, underneath all heavy equipment, inside toilet rooms and in walk in cooler with excessive amounts of dried food debris, dirt, flour and trash on floors. Instructed to remove, clean and sanitize.

- comments: observed several burned-out bulbs throughout prep areas. Instructed to replace and increase lighting throughout the premises.

- comments: no food handler training certificates on site. Instructed to obtain and maintain on site for review.
