- comments: observed evidence of pest and insect in the following areas: approx 15-20 mouse droppings behind the deep freezer and the prep table cooling unit in the rear prep area. . Approx 5 droppings by the hot water unit. . Approx 10-15 between the two door reach in freezer and the one door cooling unit. . Observed approx 10 ants on the wall and floor between the one compartment sink and the three compartment sink. Instructed to clean and maintain all areas, instructed to contact pest control and have service before return inspection date of 11/21/2025 with pest control service sheet provided. Priority foundation violation 7-38-020(b) citation issued.

- comments: observed cooking equipment with a build up of grease on the sides. Instructed to clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed a build up of items on the floor of the rear storage closet area. Instructed to organize and elevate items from the floor at least six inches as a means of being able to properly clean and prevent possible harboring space for pest and or insects.
56. Adequate ventilation & lighting; designated areas used
- comments: observed heavy build up of grease and dust on the hood ventilation system and filters. Instructed to clean and maintain.
58. Allergen training as required
- comments: observed no allergen training on site for nakeisha brooks. Informed that all city of chicago foodservice managers must obtain and provide on site.
60. Previous core violation corrected
- comments: observed previous core violations still not corrected from inspection date of 9/5/2025 with correct by date of 10/15/2025 inspection #2623363 violation #47 4-501. 11 observed damaged gasket on prep cooler door. Instructed manager to repair and maintain and violation #55 6-501. 11 observed stained ceiling tiles in prep area. Instructed manager to replace and maintain. Instructed to repair all by return inspection date of 11/21/2025 priority foundation violation 7-42-090 citation issued.
